This form sends your name, selected document type, and files to My Energy Savings for enrollment review. Your date of birth and the last four digits of your Social Security number are collected only for the document types that request them.

Your submission is stored privately in this website’s hosted storage and is accessible to authorized My Energy Savings staff. Documents and the details you provide are not publicly accessible.

Submissions remain stored until authorized staff permanently delete them. To request removal or discuss your submission, contact the agent who directed you here and provide your receipt number.
